The American Society of Pediatric Neurosurgeons (ASPN) and the Hydrocephalus Association (HA) are proud to introduce a new tool for medical providers to use with transitioning hydrocephalus patients and their families. 

As you know, a key challenge in the transition from pediatric to adult medical care is the transmission of the pediatric provider’s knowledge of the patient’s history to the adult provider. The Medical Summary for Transitioning Hydrocephalus Patients is designed to allow pediatric providers to easily summarize the key hydrocephalus-related information that receiving medical professionals need to know. It can also serve as an educational tool to use with your patients. The Summary was created through a collaborative effort between HA and the ASPN that included wide consultation with both pediatric and adult providers.

We encourage adoption of this Medical Summary template in your practices. It is available for download on both the HA and ASPN websites.
